Early Childhood Development Program (Sparkler)
Supporting your child’s early development (from birth – 5)
Description:
To help support your child’s Well Child Visit schedule for those first five years, we have partnered with Sparkler and Help Me Grow to screen for delays in early development.
This program is to support your child’s development and all screenings completed through this program will be sent to your child’s PCP so it can be reviewed during their next well child visit at CHCGD. The screeners utilized in this program are Ages & Stages Questionnaires (ASQs).

Steps to the program:
1. Download the Sparkler: Play for Parenting app (links to download below)
2. Open the app and tap “Create a New Account”
3. Enter the access code that corresponds to the office location you/your child visits:
Alex Central Health Center – OHACHC
Chambersburg Health Center – OHChamHC
Charles Drew Health Center – OHCDHC
Corwin Nixon Health Center – OHCNHC
East Dayton Health Center – OHEDHC
4. Set up (enter your email and password):
5. Create account (provide your name and contact information)
6. Add a child (their name and birthdate) – you can add additional children later
7. Complete profile questions
8. Review Program Partners and Privacy Policy
Done! Your account is created – now you will have the ability to complete the age appropriate developmental screener for your child(ren). The app will provide instruction on how to do this.
